Our Insurance and Payment Information
Arete Rehab is considered ‘Out-of-Network’ for all insurance companies except for traditional Medicare. As an ‘Out-of-Pocket’ or ‘Cash-Based’ practice, I can provide a ‘Super Bill’ for you so that your insurance carrier can reimburse you directly for a percentage of out-of-pocket expenses as per the Out-of-Network benefits and coverage, even when payment is due at the time of service.

For those using Medicare, it's crucial to understand your benefits, co-pays, and deductibles. While I'll perform an eligibility check, being informed about your insurance benefits is beneficial. Note that certain services at Arete Rehab, like massage, cranial sacral treatments, and dry needling, are not covered by Medicare. An advanced beneficiary notice will be signed at your initial visit to acknowledge these exclusions.
